Background

Large disease burden caused by heart failure among the elderly population in Hong Kong.

In Hong Kong, local hospital reported that more than 90% of patients admitted for heart failure are aged 60 years or above. Being able to identify heart failure and other cardiovascular diseases at an early stage will benefit the elderly population.

While the number of individuals aged 65 years or above is expected to increase to 2.6 million by 2040, this indicate that the disease burden caused by heart failure can be reduced if appropriate intervention is provided.

We plan to include more than 100 DECCs and NECs from 18 districts in Hong Kong participated in our project.
